For Skript developper, you can get an Item via it file path.
For exemple, if I want to set my item variable:

and my target item called:

I willl use the function : 

set {_item} to getItem("plugins/ItemCreator/Items/emerald_sword.yml")

After, you can get name, lore or type with his own attribut : 

{_item}'s name
{_item}'s type
{_item}'s lore
