Added a monitor so it watches who places Wither skeletons onto soul sand or soul soil. If a Wither is created, and there is a person nearby who was marked as having recently placed the skull on soul sand/soil, then it solely reports them and ignores other bystanders. If somehow the timing was off and it cannot determine based on skull placement, it falls back to reporting on all players nearby.
